How to Apply for Employees Health Cards Online 

ఉద్యోగులు ఫోటోలను మరియు వారిపై ఆధారపడిన కుటుంబసబ్యుల ఫోటోలను అప్ లోడ్ చేయని విషయం గమనించడమైనది. అటువంటి ఉద్యోగులు www.ehf.gov.in అనే వెబ్ పోర్టల్ ద్వారా లాగిన్ అయ్యి తమ ఎంప్లాయి ఐ.డిని (employee ID) యూజర్ ఐ.డి.ని మొదటిసారి పాస్ వర్డ్ గా వినియోగిస్తూ ఫోటోలను అప్లోడ్ చేసుకోవచ్చును.  ఈ ప్రక్రియలో ఉద్యోగులు, తమపై ఆధారపడిన కుటుంబ సభ్యుల వివరాలను కూడా అప్ డేట్ చేసే వీలు కలదు. ఇందుకు అనుసరించ వలసిన విధానం ఈ క్రింద ఇవ్వబడినది.

ఆధారపడిన కుటుంబసభ్యులు

ఫొటో: 45: 35 మిల్లీమీటర్ల కొలతతో (ఐసిఎవో తరహ) పాస్ పోర్ట్ సైజు కలర్ ఫోటోను జతచేయండి. ఇది 20౦ కె.బి కంటే తక్కువ సైజుండాలి.ఉద్యోగి జీవిత భాగస్వామి కూడా రాష్ట్ర ప్రభుత్వోద్యోగి లేదా సర్వీస్ పించను పొందుతున్న వారైతే దరఖాస్తులోని నిర్ణీత ప్రదేశంలో ఆ వివరాలను నమోదు చేయాలి.జనన ధృవీకరణ సర్టిఫికెట్‌: ఐదేళ్ళ లోపు వయసున్న కుటుంబసభ్యులున్నట్లయితే వారి జనన ధృవీకరణ సర్టిఫికెట్లను స్కాన్‌చేయండి.కుటుంబ సభ్యుల్లో ఎవరికైనా అంగవైకల్యం వున్నట్లయితే వికలాంగ ధృవీకరణ పత్రాన్ని స్కాన్‌చేయండి.

లబ్దిదారులకు ఈ పధకం ప్రయోజనాలు వెంటనే అందేందుకు అనుగుణంగా, అర్హులయినవారికి తాత్కాలిక ఆరోగ్యకార్డులను జారి చేయడం జరుగుతుంది. ఆన్ లైన్ ద్వారా సమర్పించబడిన దరఖాస్తులను డాక్టర్ ఎన్ టి ఆర్ వైద్య సేవా ట్రస్ట్ పరిశీలన పూర్తికాగానే లబ్దిదారుల లాగిన్లలోనే ఒక తాత్కాలిక ఆరోగ్యకార్డును జతచేయడం జరుగుతుంది. ఇంటర్నెట్ సాయంతో లబ్దిదారులు ఈ డిజిటల్ కార్డులను పొంది. వాటిని ప్రింట్ తీసుకొని, లామినేషన్ చేయించుకుని తాత్కాలికంగా ఉపయోగించుకోవచ్చు. అలాగే, మీసేవ కేంద్రాల్లో (డైరెక్టర్, ఇఎష్ డి, ఐటి&సి పేర్కొన్న ప్రకారం) రు.25కి మించకుండా రుసుము చెల్లించి కూడా ఈ తాత్కాలిక ఆరోగ్యకార్డులను పొందవచ్చు.తాత్కాలిక ఆరోగ్యకార్డును పొందినవారందరూ ఎంపిక చేయబడిన ఆసుపత్రుల్లో చికిత్సలు పొందేందుకు అర్హులు. ఈ కార్డు వుంటే ఈ ఆసుపత్రుల్లో చికిత్సను వెంటనే ప్రారంభిస్తారు. డాక్టర్ ఎన్ టి ఆర్ వైద్య సేవా ట్రస్ట్ ఎంపిక చేసిన ఆసుపత్రుల వివరాలను www.ehf.gov.in వెబ్ సైట్లో గమనించవచ్చు..

First call 104 and Tell your Name District ,Mandal, Treasury Id, Mobile Number, Mail Id and they will send your password

STEPS TO ENROL FOR HEALTH CARD (EMPLOYEE)

BEFORE LOGIN, KEEP THE FOLLOWING DOCUMENTS/PHOTOS READY:

SELF (Scanned copies) :

• Aadhar card/Aadhar enrolment receipt
• 1 & 2 pages of old Service Register or 4 & 5 pages of new Service Register where name of employee, date of birth , signature of head of office are available.
• Date of birth certificate (not mandatory)
• Digital copy of ICAO compliant Passport size photo (45 mm x 35 mm)
• Disability Certificate, if disabled

DEPENDENTS (Scanned copies) :

• Aadhar card/Aadhar enrolment receipt
• Digital copy of ICAO complaint Passport size photo (45 mm x 35 mm)
• Birth certificate in case of children below 5 years.
• Disability Certificate, if disabled
• Whether spouse is a government employee/Pensioner Need to add the service register/Pension Payment Order scanned copies.

NOW LOG IN TO: www.ehf.gov.in

• Click on Sign-in (Top right side of the screen)
• Your user ID is: your Employee ID( available with your DDO)
• Password is also: 104 ki call chesthe cheptharu
• Login as: Select the Employee/Pensioner from the desired dropdown list.

After login: change your password ( Length of the new password is minimum “3” characters and maximum “8” characters is mandatory, out of which one should be an alphabet, one numerical number and one special character such as “@,#,$”etc).

• Enter your ‘mobile number’
• Enter your ‘e-mail ID’.
• New password will be sent to your mobile through “SMS”/through e-mail
• After you sign-in: Click on "Registrations" at the left side
• Then click on : "Initiate Health Card"
• Now click on : Aadhar No/ Aadhar Enrolment No, as the case may be
• Enter Aadhar number / Aadhar Enrolment number as indicated below:
• a. If you have Aadhar Card enter the 12 digit number of in aadhar number box.
