Medical Reimbursement Proposal should be sent within 6 Months from the Date of discharge. The procedure and required forms are given below. The Bill resulting Below Rs. 50000 should be sent to the concerned District Educational Officer and the Bills above Rs. 50000 should be sent to the C&DSE directly through Online Mode.
మెడికల్ రీయంబర్స్మెంట్ సమాచారం సంబంధిత ఉత్తర్వులు:
1. ప్రభుత్వ,పంచాయతీ రాజ్ ఉపాధ్యాయులు మరియు పెన్షనర్లకు నిర్ధారింపబడిన వ్యాధులకు ప్రభుత్వ గుర్తింపుపొందిన ఆసుపత్రులయందు చికిత్సకై రీయంబర్స్మెంట్ విధానం వర్తించును. (G.O.Ms.No.74 తేది:15-03-20052 ఉద్యోగులకు,పెన్షనర్లకు వారిపై ఆధారపడిన కుటుంబ సభ్యులకు 2 లక్షలకు మించకుండా రీయంబర్స్మెంట్ సౌకర్యం కల్పించబడును. G.O.Ms.No.397 తేది:13-11-2008)
3. కేంద్రప్రభుత్వ ఆరోగ్య పథకం(CGHS) లో నిర్ణయించబడిన ప్యాకేజి ప్రకారం రీయంబర్స్మెంట్ ఖర్చులు చెల్లిస్తారు.
4. వైద్యఖర్చులు రూ.50,000 అయితే సంబంధిత జిల్లా విద్యాశాఖాధికారి(DEO) అంతకు మించినచో డైరెక్టర్ ఆఫ్ స్కూల్ ఎడ్యుకేషన్(DSE) కి పంపాలి.
5. ప్రైవేట్ రెఫరల్ గుర్తింపుపొందిన ఆసుపత్రులనందు కూడా 10% కోత లేకుండా పూర్తిమొత్తం చెల్లిస్తారు.(G.O.Ms.No.68 తేది:28-03-2011.
6. కీమోథేరపీ,రేడియోథేరపీ,డయాలసిస్, క్యాన్సర్,కిడ్నీ,గుండె జబ్బులు,ఎయిడ్స్,నరాల సంబంధిత వ్యాధులకు రెఫరల్ ఆసుపత్రులయందు అవుట్ పేషంట్ వైద్యఖర్చులు కూడా చెల్లిస్తారు.
7. కంటి చికిత్స,దంత చికిత్సలకు గరిష్ఠంగా రూ.10,000 చెల్లిస్తారు. కాస్మోటిక్ డెంటల్ సర్జరీకి రీయంబర్స్మెంట్ సౌకర్యం లేదు.
8.దంతచికిత్స సర్వీసులో (లేదా) జీవిత కాలంలో 3సార్లు చేయించుకోవచ్చును.ఎమర్జన్సీ సర్టిఫికెట్ అవసరం లేదు.(G.O.Ms.No.276 M&H తేది:11.05.1993)
9. రోడ్డుప్రమాదాలు సంభవించినపుడు మాత్రమే ప్రాణాపాయ రక్షణ నిమిత్తం దగ్గరలోని ప్రభుత్వ గుర్తింపులేని ఆసుపత్రులలో చికిత్స పొందినను రీయంబర్స్మెంట్ చెల్లిస్తారు.స్థానిక పోలీస్ స్టేషన్లో నమోదయిన FIR కాపిని జతచేయాలి. (G.O.Ms.No.175 తేది:29-05-1997)
10. 40సం॥ నుండి రిటైర్ అయ్యేవరకు (లేదా) జీవితకాలంలో 3 సార్లు మాస్టర్ హెల్త్ చెకప్ చేయించుకునే అవకాశం కలదు. (G.O.Ms.No.105 తేది:09-04-2007)
11. మహిళా ఉద్యోగుల తల్లిదండ్రులు పూర్తిగా డిపెండెంట్స్ అయినచో రీయంబర్స్మెంట్ అవకాశం కలదు. (DSE Rc.No.350/D2-4/2008 తేది:15-04-2008) A.S
12. కుటుంబ పెన్షన్ పొందేవారికి కూడా రీయంబర్స్మెంట్ సౌకర్యం వర్తించును.అయితే వారిపై ఆధారపడిన కుటుంబ సభ్యులకు వర్తించదు. (G.O.Ms.No.87 తేది:28-02-2004)
13. హాస్పిటల్ నుండి డిశ్చార్జి అయిన తేది నుండి 6 నెలలలోపు, చనిపోయినచో 8 నెలలలోపు DEO/DSE కి ప్రతిపాదనలు పంపాలి.
